Florida High School Rodeo Association
2009-2010
Welcome!
Thank you for your interest in the Florida High School Rodeo Association. Below is some information to help you get started. Our first rodeo is in late August, but any high school student can join any time throughout the year.
On this page, you will find the membership applications for the Florida High School Rodeo Association for the 2009-2010 Rodeo year. These forms must be completed and returned to the State Secretary along with the correct amount of fees. Fees may be paid by personal check or money order made payable to FHSRA.
IMPORTANT NOTICE FOR RENEWING MEMBERS: In order to reduce the amount of paper work to be completed at the first rodeo, renewing members MUST HAVE their applications returned postmarked NO LATER THAN AUGUST 12, 2009, or they will be accessed a $50.00 Penalty Fee and will not be able to participate in rodeos until that fee is paid. New members will not be penalized, but are strongly encouraged to return their papers for processing as soon as possible BEFORE the first rodeo. Forms cannot be faxed; they have to be mailed to me.
Competing student membership dues are $254.00. These dues include the National High School Rodeo Assn dues, secondary insurance, a subscription to both the NHSRA Times and Western Horseman magazine, and the FHSRA Family membership dues. Enclosed you will find a Family Membership Form. Please list all family members who will be attending the rodeos this season. Non-contestant membership cards will be available to all FHSRA family members. The cost of the family membership has been included with the contestant’s State dues this year.
.
There are two (2) release forms, the NHSRA and FHSRA. Please make certain that both of these forms are completed and notarized. Both parents and the student’s signatures must be notarized on both forms. Please note there are 3 areas that require signatures, including the bottom of the National High School Rodeo Assoc. Membership Application, many of you forgot to sign the front page last year. The Notary Public must witness and list by name the parents and the student on each form. If you do not live with both parents, please mark either ‘deceased’ or ‘full custody’, as appropriate. If this is not done, the forms will be returned to you for completion. Also, be sure to include the student’s Social Security number where indicated. This is imperative because the NHSRA will not recognize your membership or insurance activation until your application is complete.
All new 9th grade members must include with their application a copy of their birth certificate and their last report card indicating they are entering the 9th grade this fall. Renewing members also must include a copy of their last high school report card. Home schoolers must submit proof of testing from a certified teacher as proof of performance at grade level. The Report Card Form must be completed.

Once again, I can’t stress enough that all forms in this packet be completed and returned to me as soon as possible. Each member will receive a membership packet, and a copy of the 2009-2010 Florida ground rules at the first rodeo. New members will also receive a copy of the NHSRA rulebook. Returning members received a new two-year rulebook in your folders last season. If you need a replacement rulebook please see me at the first rodeo check in. It is very important that you familiarize yourself with both the NHSRA rulebook and the Florida 2009-2010 Ground Rules.
Our annual membership meeting has been set for August 8, 2009 at the Fat Boys Bar BQ in St. Cloud at 11am. This meeting is where most of the decisions will be made for this rodeo season. It is open to the general membership and we encourage you and your family to come and get involved. Please watch our web site for updates at www.fhsra.homestead.com. I look forward to working with you all again this year. Thank you so much for all your help, support and cooperation. More information will be coming soon. Please call me if you have any questions. Your directors and officers are looking forward to making this a great year!!

Be sure to fill out the top portion of the application COMPLETELY. Make sure everyone signs the application. If the parents are divorced I still need both parents to sign. If one parent is deceased or one parent has full custody, LIST THAT WHERE THEY WOULD SIGN.
Fill out completely. Next step is very important!!!
When signing the release and having it notarized, everyone needs to sign the release form. That means mother, father, and member. If divorced full custody or deceased list that in their space. The notary must list all three names she/he notarizes the form.
